Today, we will discuss about the Circumcision cpt code 54150, 54160 and 54161. CPT code 54160 abd 54161 is coded based on the age of the patient. CPT code 54150 specify the types of penile nerve block provided for neonatal circumcisions, reflecting a change in clinical practice in newborn circumcisions.
Circumcision codes 54150-54161 are divided based on whether the circumcision was accomplished by means of a clamp/other device or surgical excision and whether the procedure was performed on a neonate or non-neonate. A clamp is a device that is used to restrain the foreskin of the penis while the skin is trimmed. Report newborn circumcisions that utilize a clamp or other device with 54150. Surgical excision of the foreskin is a procedure in which a clamp or other device is not used, and the surgeon directly excises the skin from the penis. Once the skin has been removed, the incision is closed with sutures. Report the surgical excision without the use of a clamp or other device with 54160 (neonate) or 54161 (except neonate).

Description of Circumcision cpt code 54150, 54160 and 54161
In this procedure the physician removes the foreskin of the penis in a baby up to 28 days of age (CPT code 54160) or in a male older than 28 days (CPT code 54161) by excision of the skin. Then a segment of foreskin on the dorsal or the side of the penis is crushed with forceps.
A cut is made through the crushed tissue with scissors, and the divided foreskin is pulled down over the head of the penis while the excess skin is trimmed from around the head of the penis. Bleeding is controlled by chemical cautery or suture ligatures. The skin edges created are sutured together with absorbable suture material.
For Procedure code 54150, the physician injects the dorsal nerve bundle and does a complete circumferential penile blockade with 1% lidocaine. The patient is observed. The patient is strapped to a papoose board and the surgical site is prepped. The foreskin ring is dilated with a hemostat and reduced to the shaft of the penis. All foreskin-glans adhesions are taken down with a tear duct probe and all smegma concretions are removed.
Reprepping is accomplished and the foreskin is placed over the glans. One of three Gomco clamps is selected for the size of the glans and inserted through the foreskin riding over the glans. The constricting circular ring is placed over the foreskin to compress and devascularize the foreskin. The device is left in place until devascularization occurs. The devascularized foreskin is excised with a scalpel, the ring device is removed, and the penis is dressed.

54160 Circumcision, surgical excision other than clamp, device, or dorsal slit; neonate (28 days of age or less)
54161 older than 28 days of age
As per the coding guidelines, do not report modifier 63 in conjunction with 54150. And assign 54150 with modifier 52 when performed without dorsal penile or ring block.
Do not report modifier 63 in conjunction with 54160.
