Basics of Pulmonary tissue ventilation analysis code 0807T, 0808T

For the CPT 2024 code set, two new Category III codes (0807T, 0808T) were established to report pulmonary tissue ventilation analysis. This article provides an overview of the intent and use of these new codes.

The provider uses software to analyze data that measures the patient’s pulmonary tissue ventilation from separately obtained cinefluoroscopic images. The software uses algorithms and mathematical models to translate images to quantitative data, which can then be used for diagnosis and treatment planning. Software analysis of the cinefluoroscopic images is done in combination with information that has been obtained from computed tomography (CT) images.

Software-analyzed data is used to show pulmonary function changes and regional ventilation, identifying areas of high and low ventilation to help determine overall lung health. Code 0807T is reported when pulmonary tissue ventilation analysis is performed using CT images that were previously acquired. Code 0808T is reported when the CT images are obtained as part of the ventilation analysis. These codes include data preparation and transmission, pulmonary tissue ventilation quantification, review of data, and interpretation and report.

New codes 0807T and 0808T were established to report pulmonary tissue ventilation analysis using software-based processing of data from cinefluorographic images. These codes include image processing technology to quantify the distribution of ventilation of pulmonary tissue.

The following clinical examples and procedural descriptions reflect typical clinical scenarios for which these new codes would be appropriately reported.

Clinical Example (0807T)

A 72-year-old male with COPD is no longer adequately controlled with medical therapy. His physician orders pulmonary ventilation analysis using software-based processing of data from cinefluorographic images in combination with previously obtained CT images.

Description of Procedure (0807T)

Obtain five cinefluorographic images of the lung for the purpose of pulmonary ventilation analysis, and upload images to an image routing system. (Acquisition of these images is included in the code and not separately reported.) The image routing system then identifies corresponding images from a previously obtained CT scan of the chest. The images are then transmitted to the analysis engine via standard digital imaging communications in medicine protocols for pulmonary ventilation analysis. Perform pulmonary ventilation analysis using specialized analysis software and advanced engineering computation. All results from the pulmonary ventilation analysis are included in a structured quantitative report. Send the results report to a QHP for review and interpretation. The QHP considers all data in the results report and dictates a medical report.

Clinical Example (0808T)

A 73-year-old male with COPD is no longer adequately controlled with medical therapy. He undergoes a CT of the chest. His physician orders pulmonary ventilation analysis using assistive software-based processing of data from cinefluorographic images in combination with concurrently obtained CT images.

Description of Procedure (0808T)

Obtain five cinefluorographic images of the lung and CT of the chest for the purpose of pulmonary ventilation analysis, and upload images to an image routing system. (Acquisition of these images is included in the code and not separately reported.) The image routing system identifies corresponding images between the cinefluorographic exam and CT and transmits these images to the analysis engine via standard digital imaging communications in medicine protocols for pulmonary ventilation analysis. Perform pulmonary ventilation analysis using specialized analysis software and advanced engineering computation. All results from the pulmonary ventilation analysis are included in a structured quantitative report. Send the results report to a QHP for review and interpretation. The QHP considers all data in the results report and dictates a medical report.
