Medical and Surgical Section (0): Two Body System Options
Within the Medical and Surgical section, coders can report cranial procedures using one of two body systems:
Central Nervous System and Cranial Nerves (0)
Some tables list only general body part values, such as Brain (0) and Cranial Nerve (E). These general values apply when they’re the only option in a table row, or when no more specific site is documented. For instance, body part value Brain (0) is the sole option in the first four rows of Insertion table 00H.
For structures without a dedicated character, Appendix D (Body Part Key) and Appendix E (Body Part Definitions) identify the correct value to assign. Key examples include:
- Brain (0): Encephalon (the entire central nervous system inside the skull), cerebrum, and corpus callosum
- Cerebral Meninges (1): Intracranial arachnoid mater, leptomeninges, pia mater
- Cerebral Hemisphere (7): Frontal, occipital, parietal, and temporal lobes
- Cerebellum (C): Includes the culmen
Coding Intracranial Dura Mater Spaces
The layers of intracranial dura mater don’t normally have spaces between them, but trauma or pathology can create fluid-filled spaces (blood, cerebrospinal fluid, or pus). Use these body part values with root operations Drainage (9), Extirpation (C), or Fragmentation (F):
- Epidural space, intracranial (3)
- Subdural space, intracranial (4)
- Subarachnoid space, intracranial (5)
Coding Nerves Within the Brain
Nerves inside the brain fall into two groups:
Deep brain and base-of-brain structures:
- Basal ganglia (8)
- Thalamus (9)
- Hypothalamus (A)
- Cerebellum (C)
Numbered cranial nerves: values F–H and J–S. Two of these nerves extend beyond the brain and are coded the same way regardless of where the procedure occurs:
- Vagus Nerve (Q): The tenth cranial nerve, running from the brain through the neck, chest, and abdomen
- Accessory Nerve (R): The eleventh cranial nerve, running from the brain into the spinal root
If a procedure targets a nerve branch without its own body part value, 2026 ICD-10-PCS Guideline B4.2 directs coders to the closest proximal branch that does have a specific value. For example, procedures on the mandibular branch are coded to Trigeminal Nerve (K).

Under this body system, Cranial Cavity (1) is the only body part value for intracranial procedures. Applicable root operations include:

Cranial Cavity Coding for Other ICD-10-PCS Sections
Imaging Section (B): Brain Imaging Codes
Under body system Central Nervous System (0), body part values include Brain (0), Cisterna (7), and Cerebral Ventricle(s) (8). Root operations cover CT Scan (2), MRI (3), and Ultrasonography (4).
Nuclear Medicine Section (C): Brain Nuclear Medicine Codes
Under body system Central Nervous System (0), body part values include Brain (0), Cerebrospinal Fluid (5), and Central Nervous System (Y). Root operations include Planar Imaging (1), Tomographic Imaging (2), PET Imaging (3), and Nonimaging Nuclear Medicine Probe (5).
Radiation Therapy Section (D): Brain Radiation Therapy Codes
Under body system Central and Peripheral Nervous System (0), the only body part value is Brain (0). Root operations include Beam Radiation (0), Brachytherapy (1), Stereotactic Radiosurgery (2), and Other Radiation (Y).
New Technology Section (X): Brain-Related Body Systems
Three body systems in New Technology apply to the brain:
- Nervous System (0): Body part values Brain (3), Vagus Nerve (Q), and Prefrontal Cortex (0). Root operations include Destruction (5), Insertion (H), and Other Procedures (Z).
- Anatomical Regions (W): Body part value Cranial Cavity and Brain (Q). Root operation: Introduction (0).
- Physiological Systems (X): Body part value Central Nervous (0). Root operations include Monitoring (2) and Measurement (0).
How to Code Common Cranial Cavity Procedures
Coding Bore Hole (Burr Hole) Procedures
A bore hole is drilled into the skull to relieve intracranial pressure from fluid or hematoma, treat abscesses, or provide access for other brain procedures. Since a bore hole is an approach rather than a standalone procedure, the code depends on the underlying purpose of the surgery.
How to code it: Report a code from Anatomical Regions, General (W), body region Cranial Cavity (1), with Percutaneous Approach (3). Applicable root operations include Drainage (9), Fragmentation (B), or Extirpation (C).
Coding Bypass Procedures of Brain Nerves
Nerve bypass procedures reroute nerve signals around damaged tissue to restore mobility and sensation in paralyzed patients.
Per ICD-10-PCS Guideline B3.6a, bypass procedures require identifying the body part bypassed “from” (fourth character) and the body part bypassed “to” (seventh character, qualifier). Qualifier values include:
- Nasopharynx (0)
- Mastoid Sinus (1)
- Atrium / Blood Vessel (3)
- Pleural Cavity (4)
- Intestine (5)
- Peritoneal Cavity (6)
- Urinary Tract (7)
- Bone Marrow (8)

Coding Hydrocephalus Shunt Placement
Shunting for hydrocephalus is also classified as a bypass procedure. A flexible tube drains excess cerebrospinal fluid from the brain’s ventricles to another area of the body, most commonly the abdomen.
Brain-to-abdomen shunt coding: Use table 001 with body part Cerebral Ventricle (6), the correct approach and device values, and qualifier Peritoneal Cavity (6). Remember: the body part value reflects the tissue bypassed from, while the qualifier reflects the tissue bypassed to. Report separate codes whenever different device values are used.
Coding Facial Nerve Transfer Procedures
Transfer procedures address facial paralysis. Both the body part and qualifier values draw from the numbered cranial nerves 1 through 12.
Coding Brain Mapping Procedures
Mapping traces electrical impulse pathways and/or identifies functional brain regions. Table 00K lists body part values Brain (0), Thalamus (9), Hypothalamus (A), and Cerebellum (C), with qualifier values No Qualifier (Z) and Connectomic Analysis (1).
Connectomic analysis generates images that map the location and direction of white matter bundles, helping identify connectivity issues such as those associated with autism.
Coding Neurostimulator Insertion for the Brain
Neurostimulator devices deliver electrical pulses to targeted brain regions to treat movement disorders, including Parkinson’s disease, tremors, epilepsy, and obsessive-compulsive disorder. The device includes leads placed in the brain, wires under the skin, and a battery-powered pulse generator.
This procedure requires two codes:
- Pulse generator insertion: A code from Subcutaneous Tissue and Fascia (J), using device value Stimulator Generator (B–E, M)
- Lead insertion: A code from Central Nervous System and Cranial Nerves (0), using device value Neurostimulator Lead (M)
Coding Brain Destruction Procedures
Table 005 (Destruction) includes a row for the general body part value Brain (0), plus rows for specific nerves such as Cerebellum (C) and Glossopharyngeal Nerves (P). For Brain (0), qualifier values identify specific therapies:
- Laser Interstitial Thermal Therapy (3): Uses focused ultrasound with a chemical agent to treat cancer
- Stereoelectroencephalographic Radiofrequency Ablation (4): Uses radiofrequency energy to create lesions in patients with drug-resistant epilepsy
If a blood-brain barrier disruption is performed during a Destruction procedure, code it separately using the Administration section (3), body system Physiological Systems and Anatomical Regions (E), root operation Introduction (0). The substance is administered via body part value Peripheral Artery (5) or Central Artery (6), with qualifier value Blood Brain Barrier Disruption (N).
Coding Brain Procedures in Imaging, Nuclear Medicine, and Radiation Therapy
- Imaging Section (B): Used for creating images of the brain or cranial cavity using instruments outside the body.
- Nuclear Medicine Section (C): Used when an unsealed radioactive substance is introduced into the body to create diagnostic images or treat conditions like cancer or Alzheimer’s disease. Report a separate code for each radiopharmaceutical administered.
- Radiation Therapy Section (D): Used when sealed (encapsulated) radionuclides — placed inside or outside the body — treat cancer.
Coding Brachytherapy for Brain Cancer
Brachytherapy places a solid radioactive substance inside the body near a tumor and is reported with modality value 1.
If the radioactive element remains in the body after the procedure, two codes are typically required:
- Table 00H (Insertion): Reports the radioactive element using device value Radioactive Element (1)
- Table D00 (Radiation Therapy): Provides additional detail, such as dose rate and isotope used
Exceptions: Cesium-141 Collagen Implant (4) and Palladium-103 Collagen Implant (5) are device values found only in table 00H.
Other brachytherapy coding rules:
- When brachytherapy is administered, use a Radiation Therapy section code.
- When a temporary applicator is inserted for later brachytherapy delivery, use table 00H with device value Other Device (Y).

Coding Brain Procedures in the New Technology Section
New Technology section codes apply to highly specific procedures and substances:
- Focused Ultrasound Therapy (treats cancer via low-intensity ultrasound and a chemical agent): Body system Nervous System (0), root operation Destruction (5), body part Brain (3), device/substance/technology value 4. If applicable, also code any blood-brain barrier disruption or other therapeutics administered during the same session using the Administration section (3).
- Computer-Aided Detection and Notification (monitors brain electrical activity): Body system Physiological Systems (X), root operation Monitoring (2), body part Central Nervous (0), device/substance/technology value 8.
